    root canal treatment(RCT)-dental education

    Dear,jatlanders...
    ..Dental pain or toothache is most annoying and is highly painful condition that manytimes keep u awake overnight and comes 2nd painful condition after labourpain..it is so painful that in past patients hv attempted suicide when there was no treatment available for dental pain..

    ..Root canal treatment is the among the most common dental treatments and most of the time is the treatment of choice when u feel dental pain or ur tooth is sensitive to hot or cold....
    ..next treatment of choice is the removal of tooth and sometimes the only option when the tooth is badly decayed and RCT is not possible..
    ..a general dentist can call u 3-4 times for single root canal while an endodontist can perform it in single sitting(40 min-1 hr)
    ..i m sure many of the jatlanders reading it must hv root canal done to their teeth nd had different experience with it..

    ..one misconception is that root canal procedure is higly painful,root canal procedure is never painful when u get it done by a root canal specialist(endodontist)..root canal procedure is highly technique sensitive and a general dentist is not so trained in performing root canal...
    ..i m sharing with u one animated video that shows why your tooth require root canal and how it is performed...
    ..cost of one root canal treatment in foreign countries e.g U.S is 1,200-2,500 u.s dollars(around rupees 70,000-1.2 lac).. and in india it varies between rupees 2000 - 10,000.
    ..an endodontist on an everage perform 4-5 root canal per day,so an endodontist in foreign coutries earns around 8-10,000 us dollars( 4-5 lac rupees per day) and in india 20-30 thousand rupees per day..(sad)

    NOTE: my sole purpose if starting this thread is to aware all the jatlanders about sm of the common dental procedure..so that if next time u visit a dentist u be money and mind prepared..and also doesnt fall in hands of malpractitioners nd unethical dentists..

    Dr. Karminder,

    Appreciate your gesture in starting a dental care thread. I have some questions - not on root canal. But would be great if you can answer. I and my wife got a dental health check up and cleaning done at a private clinic (BUPA) in UK recently. This is the first time I went to a dentist without having an issue (just because I had insurance). After cleaning, she advised both of us to visit a dental hygienist 3 times in next 12 months saying the teeth are fine, but we have issues with gums (plaque?) - and booked the first appointment the same week. We discussed later and missed the appointments. Basically, private dental care is a bit of a scam here and there are reports of dentists asking for more regular check-ups/cleaning than necessary. We didn't like the experience we had and can't trust the diagnosis.
    - Couldn't she have done the first round of hygienist part during "cleaning"? [She had atleast 30 minutes extra time based on what she was charging]. We didn't understand why we need to go through another round of cleaning the same week.
    - Are such regular "hygiene" visits required? She said that there is a lot of gum cleaning to be done. That it won't happen in one go. While these would be covered, we don't want people fiddling with the teeth when not required. Is it possible to have no discomfort and still have bad gums?
    - She also advised me to get a filling done again - something I had got done a long time back in India. I have no discomfort there and would rather get it done during an India visit. Can it deteroirate suddenly?

    I have paid 900 dollar for root canal.450 dollars my insurance paid rest i had to give from my pocket.

    here there is separate doctor for gum surgery.after the root canal is done and before putting the crown they do some kind surgery on gum around the tooth.for this gum surgery also i paid more than 500 dollar.for putting the crown my dentist charged more than 500.three doctors were involved .i had to pay almost a 1000 dollar from my pocket.my dentist suggested me to get all my wisdom teeth removed .I had to pay 2000 dollar from my pocket.and for tooth extraction there is different kind of doctor.bhagwan bachaye usa ke dentisto se.luckily ab kai saal ho gyye.thanks to flossing and turmeric and salt gargles.floss is a must to have healthy gums.

    RCT to maine bhi karayi thi...sirf 2000 rupees main..except cap cost...per wa RCT ki filling e na dat ti...her mahiney likad jya hai...doctor saying ki space nahi hai filling ko hold karne ke liye....Any body have some solution (Daant nikalwane ke alawa)

    .dear kapil...gums cleaning(oral prophylaxis or scaling) is a broad treatment and cover many minute treatments under it..

    ..there are two types of gum cleaning...one is supragingival(above gum line that can be easily cleaned at one appointment)
    ..next is subgingival(that is hidden and below gums and require 3-4 recalls and follow up)....supragingival cleaning is done by dental hygienist..
    ..but they are not authorized or licensed to do subgingival cleaning...subgingival cleaning is done by gum specialist(periodontist) nd it also cover supragingival cleaning(first supra than sub)
    ..my only confusion is that if she has completed ur cleaning than she must hv completed ur supragingival cleaning also nd why she is refering u to dental hygienist..from this point wt i can say that she has not yet completed ur cleaning part nd is refering u to hygienist for completion of that treatment..(smtime when the supragingival deposits are more..a periodontist perform the gross cleaning nd refer to hygienst for completion and recalls for followup every 5-6 month)
    ..also u are young enough not to hv subgingival problem(mainly occurs after 40 yrs if u r practicing good oral hygiene daily)..
    ..so dont worry about that nd get all ur dental treatments done when u visit india in future....
    ..about ur decayed tooth problem i can tell u in detail after seeing the x ray of that tooth..
    ..let me know if u had any pain in that filled tooth till now(even minute nd short duration)..nd when u got that filling..it will no deteriorate so quickely nd can get it done in india..

    ..good..i m happy that i m getting the same response to this thread that i expected..jatlanders who are staying abroad should get more advantage because i can understand that they hv to think many times before visiting a dentists as dental treatment is very costly there...
    ..i hope ur treatment was done by an endodontist nd its not giving any problem now...
    ..if u hv any problem in future .please feel free to ask..

    ..one more thing if u hv ur pre and post operative x ray reports...mail me ,i will tell u the status of ur root canal treatement...
    ..ur dentist must be having those xrays in ur patient record..

    ...there is always a seperate doctor for gum surgeries..nd is known as PERIODONTIST...

    ..after RCT ..putting a cap(crown) on the tooth is must as after root canal treatment as ur blood supply to the tooth is cut and the tooth become brittle nd is prone to fracture..so once the tooth is fractured,ur RCT will go waste nd hv to remove that tooth...
    ..by knowing the cost of treatment that u hd paid nd team of three doctors ..i can say that ur tooth required crown lengthening..
    crown lenthening is actually done when ur tooth is very much decayed and there is not enough part left on top to put the cap after RCT..in that case we hv to surgically exposed sm part of that tooth by cutting the gums around it nd then place the cap on it...
    ..wisdom teeth only cause discomfort to individual..these teeth dnt help in anything as there postions are almost alwys abnormal..better to get them removed..now a days we are praticing to remove these teeth during their formative stage only before they get big enough to get removed..usually at age of 17-18 years....
    ..again i suggest u to get all ur dental treatment done during ur visit to india and indian dentist are good at surgical work....

    ..but u are really heavily charged for crown lengthening procedure and wisdom tooth extraction...

    .dear satender..if u hv no problem(pain or anything) in that tooth..i will suggest u to go for cap(crown ) on that tooth as soon as possible
    ..as u are saying that filling is not staying in that tooth. so,i expect that the tooth is badly decayed nd is in poor condition..
    ..get ur tooth capped...otherwise if it will fracture beyond management.it will go for removal...

    my question is
    what should be the frequency to visit a dentist if u don’t have any problem. Is scaling a necessity and one should do it regularly.

    ..even with utmost care of ur daily oral hygiene ,we can not prevent the accumulation of harmful deposits(plaque) on our teeth...
    ..that harmful deposits can only be removed mechanically with external aids other than regular oral hygiene practice..
    ..so evry 6 months u should visit ur dentist for checkups so that the deposits can be removed at an early stage before they start gum irritation and other problems..
    ..frankly speaking me myself being a dentist , i m using electric tooth brush with all mouth rinses solutions that required..but i still get my teeth cleaning every 6 months..
    .nd specially ppl with crowded teeth hv more tendency of plaque deposits..they should undergo full mouth prophylaxis every 6 months...

    I visit dentist two times a year for cleaning only and that is routine since last 10 years. I have never been advised by my dentist for any other jobs to be done except a thorough check up and x-ray every two years.

    Thanks for starting this thread, Dr. Karminder.

    A friend of mine says that when he eats sour, spicy or chilly-stuff or sweets (Burfi, chocolate etc.), he feels acute pain on two upper teeth (one wisdom tooth and its adjoining one). He says that in India, there is a toothpaste (name not known to him) which is recommended by dentists in such cases and with its regular use, this problem is solved to some extent. What is its name and how long it needs to be used and how many times per day?

    Thanks Dr. Karminder, really appreciate it.

    I am going to shoot her an email and ask her which cleaning she did, based on the details you told me. I think she was talking about hidden residue in gums - subgingval. Now after your explanation, I don't understand why she wants me to see a dental hygienist (and that too 3 times a year) if the hygienist isn't authorised to do it.

    The bill says "Scale and polish" for the services provided, so she might have just polished the teeth (that I never wanted in the first place). I am going to be quite pissed about it as she was done with me in less than 30 minutes, but charged me for 30 min - 1 hour of consultation + 30 min of cleaning. The shocking part is that they don't give a diagnosis - she didnt even give me my X-rays. Let me give her some hard time about supragingival and subgingival...

    She had recommended a special anti-bacterial mouthwash Curaseptic that stops formation of plaque. I got that but it says "use for 10 days. Consult dentist if you need to continue further". Is it something I can continue using? I am basically looking for non-intervention ways where I can clean my gums myself (maybe over a long period of time).

    As for filling, it was more than 5 years back. These days I sometimes feel something funny (A bit of a stretch to call it pain), but I think it is psychological since she put it in my mind. (Is it possible that she knocked off my filling while cleaning - "ki ab to aayega hi waapas" )

    Dr. Karminder, is it advisable to get this removed? I think I read a report that some X% of surgeries fail (1%?) and may lead to serious issues like paralysis. And is there a way to say when to get removed? In UK, apparently many dentists advise this for no good reason just to make the huge surgery fees.

    It is your prerogative, but I don't think anyone should feel shy about sharing their dental details - it is quite a safe area to talk about. The reason I say this is because otherwise, you can potentially be getting same sort of problems from everyone. While a public response can benefit everyone and pre-empt duplicate questions.
